CALL TO ORDER
Chair Gephart called the meeting to order at 2:10 p.m.

Andrew Apodaca attended via telephone.
CALL TO AUDIENCE - at this time, any member of the public is allowed to address the Board on any issue not
listed on today’s agenda. Pursuant to the Arizona open meeting law, individual Board members may ask Town
staff to review the matter, ask that the matter be placed on a future agenda, or respond to criticism made by
speakers. However, the Board may not discuss or take legal action on matters raised during "Call to Audience."
In order to speak during "Call to Audience", please specify what you wish to discuss when completing the blue
speaker card.
Notice of Possible Executive Session
Pursuant to ARS § 38-431.03 (A)(2)(3), the Board may vote to convene in executive session to discuss or
consider confidential records exempt by law from public inspection, and/or to consult with the Board's attorney for
legal advice and to consider the Board's position and to instruct the Board's attorney regarding its position in
contemplated litigation regarding Agenda Items 4-6. The Board will take action on these items in open session.

1.REVIEW AND APPROVAL OF THE MARCH 14, 2023 REGULAR SESSION MEETING MINUTES
Members reviewed the minutes as written with no discussion.
Motion by Member Carmen Trevizo, seconded by Member Katherine Beck to approve the March 14,
2023 regular session minutes.
Vote: 5 - 0 Carried

2.REVIEW AND APPROVAL OF THE MARCH 14, 2023 EXECUTIVE SESSION MEETING MINUTES
Members reviewed the minutes as written with no discussion.
Motion by Chairman David Gephart, seconded by Member Katherine Beck to approve the March 14,
2023 executive session minutes.
Vote: 5 - 0 Carried
3.APPOINTMENT OF OFFICE:
MAYOR AND COUNCIL APPOINTED KATHLEEN HERNANDEZ AS MEMBER TO THE PUBLIC
SAFETY PERSONNEL RETIREMENT SYSTEM LOCAL BOARD ON JUNE 8, 2023 FOR A TERM
ENDING ON SEPTEMBER 30, 2024
Motion by Chairman David Gephart, seconded by Member Carmen Trevizo to acknowledge the
appointment of Kathleen Hernandez as Member to the Public Safety Personnel Retirement System Local
Board for a term ending September 30, 2024.
Vote: 5 - 0 Carried
4.NEW MEMBERSHIP APPROVAL AND REVIEW FOR POSSIBLE PRE-EXISTING CONDITIONS FOR
NEW OFFICER BRITTNEY MENDEZ, HIRE DATE MAY 15, 2023, TIER 3 DATE AUGUST 13, 2023
Mr. Apodaca stated that unless the Board had any questions about agenda items 4-6, there was no
need to enter executive session. The Board reviewed the information and stated that they did not have
any questions for Mr. Apodaca.
Motion by Member Carmen Trevizo, seconded by Member Roger Reynolds to find that Brittney Mendez
meets all applicable PSPRS membership definitions, including the definition of Member, A.R.S. § 38-842
(31), and all its related definitions, and regular assignment to hazardous duty, pursuant to A.R.S. §
38-842 (42), effective on the date of her hire, May 15, 2023, with a Tier 3 date of August 13, 2023 AND to
declare the Board's finding of one (1) pre-existing condition, as noted in the medical examination report.
Vote: 5 - 0 Carried
5.NEW MEMBERSHIP APPROVAL AND REVIEW FOR POSSIBLE PRE-EXISTING CONDITIONS FOR
LATERAL TRANSFER OFFICER ALFONSO CARDENAS WITH A HIRE DATE OF MAY 30, 2023
Motion by Chairman David Gephart, seconded by Member Carmen Trevizo to find that Alfonso
Cardenas meets all applicable PSPRS membership definitions, including the definition of Member,
A.R.S. § 38-842 (31), and all its related definitions, and regular assignment to hazardous duty, pursuant
to A.R.S. § 38-842 (42), effective on the date of his hire, May 30, 2023 AND to declare the Board's
finding of two (2) pre-existing condition(s) as noted in the medical examination report.
Vote: 5 - 0 Carried
6.NEW MEMBERSHIP APPROVAL AND REVIEW FOR POSSIBLE PRE-EXISTING CONDITIONS FOR
NEW OFFICER MICHAEL BENTLEY, HIRE DATE JULY 10, 2023, TIER 3 DATE OCTOBER 8, 2023
Motion by Member Roger Reynolds, seconded by Member Katherine Beck to find that Michael Bentley

Motion by Member Roger Reynolds, seconded by Member Katherine Beck to find that Michael Bentley
meets all applicable PSPRS membership definitions, including the definition of Member, A.R.S. § 38-842
(31), and all its related definitions, and regular assignment to hazardous duty, pursuant to A.R.S. §
38-842 (42), effective on the date of his hire, July 10, 2023, with a Tier 3 date of October 8, 2023 AND to
declare no pre-existing condition(s), as set forth in the medical examination report.
Vote: 5 - 0 Carried
7.REVIEW AND APPROVAL OF RESIGNATIONS AND RETIREMENTS OF THE FOLLOWING
MEMBERS, THUS TERMINATING THEIR MEMBERSHIPS: VIVIAN LOPEZ RETIREMENT EFFECTIVE
MAY 29, 2023, ANDREW MAISH RESIGNATION EFFECTIVE JUNE 22, 2023
Motion by Member Carmen Trevizo, seconded by Member Roger Reynolds to approve the resignation
and to terminate the membership of Andrew Maish AND to approve the retirement and to terminate the
membership of Vivian Lopez.
Vote: 5 - 0 Carried
ADJOURNMENT
Motion by Member Katherine Beck, seconded by Member Roger Reynolds to adjourn the meeting at
2:23 p.m.
